Northbrook Heights Assisted Living Community

Conveniently located on a quiet residential cul - de - sac, Northbrook Heights has provided care and services to the elderly and disabled of Cayuga County and beyond since 1974. It is licensed by the New York State Department of Health as an Adult Care Facility and an Assisted Living Program. Unlike many of its competitors Northbrook Heights is locally owned and operated. Unlike many of our competitors, a resident is not forced to move out of the facility once their personal financial resources are exhausted. Northbrook Heights accepts the New York State Congregate Level - 3 reimbursement rates for Medicaid eligible residents. Northbrook Heights provides two levels of care; Adult Care and Assisted Living. This allows residents to age in place, without having to move out of the community to which they have become accustomed and developed friendships when their needs change. Northbrook Heights is here to serve you. We are indeed a “Home with a Heart”

The Adult Care Facility (ACF)

The ACF program is designed to provide monitoring and the care and services listed above to more independent higher functioning residents.

The Assisted Living Program (ALP)

Our ALP program is the only New York State Licensed Assisted Living Program in Cayuga County. Residents in the ALP require and receive a higher level of care and services than in the ACF. The ALP level of care includes the development of an individualized care plan for each resident by a Registered Nurse for such assistance as transfer, ambulation, toileting, comfort checks , escorts to and from meals, and care and services for other special needs. Residents meeting income and financial resource criteria may have the cost of the ALP services covered by Medicaid. Please contact our Admissions staff for further information regarding the ACF and ALP programs.

Clinical Care Coordination

Health monitoring is provided daily by licensed nursing and social worker staff to determine residents’ needs. This includes communication with physicians and other health/medical care providers and arranging appointments, obtaining prescribed medications, obtaining urgent care or emergency care if needed, and directing personal care staff to meet the changing care needs of each resident

Recreation and Activities

Northbrook Heights offers an array of recreational activities on a daily basis including; bingo, exercises (including modified zumba), game night, happy hour, ice cream socials and a variety of entertainers. Additionally, periodic outings to places of interest are available for a nominal fee.

Northbrook Heights is fortunate to have an experienced chef who is involved in the preparation of three homemade meals each day. Residents are provided the opportunity to provide input into the development of our seasonal menus which is then reviewed and approved by a Registered Dietician. Additionally, residents are offered between meal snacks daily.
